What do I need for my first appointment?
For your first visit, please bring your completed new patient forms (can be accessed on our New Patient Resources page), your valid form of ID, a list of medications you're taking and any current medical conditions you have, and your dental insurance card (if you're covered).

What should I do if I knock out a tooth?
Pick the tooth up by the crown (not the root), gently rinse it, and try placing it back in the socket. Then, call or visit our practice as soon as possible. If the tooth won't go back in the socket, store it in milk and then visit or contact us ASAP.
